Individuals who need all Services can:

Self-refer by contacting a Housing Stabilization Services provider directly.

Be referred by a provider or case manager (including targeted or waiver case managers).

Information Needed for Referral

To determine eligibility, please provide the following documents:

✅ Assessment (MNChoices, Professional Statement of Need (PSN), or Coordinated Entry) completed within the last 12 months.

✅ Proof of disability (PSN, medical opinion form, or SSI/SSDI recipient status).

✅ Person-Centered Plan (Community Services and Support Plan, Coordinated Care Plan for seniors, or Housing-Focused Person-Centered Plan).

Metro Housing Support Inc. will submit the eligibility request to DHS for review. Once approved, DHS will notify both the provider (via MN−ITS mailbox) and the individual (via U.S. mail) of the decision.
